Welcome to our Online Patient Portal!

Why call when you can click? Our Patient Portal is a web-based system that is your secure communication link with our office. When you log in to the Portal with your private user name and password, you can:

• Use the messaging feature to contact us.
• Request a medication refill.
• View results of lab and other diagnostic tests.
• View, print, or save an electronic copy of your Health Summary.

Note: To get a Username and Password, please print the consent form below and bring the completed form (or mail it) to our office. We will set up your new Patient Portal account the day your form is received and email you with your log-in information.
